EYE17 Participation Release, Contact Information, and Health/Medical Forms

Three forms must be completed for every participant attending EYE17. They are:
- EYE17 Event Agreement and Release
- EYE17 Contact Information
- EYE17 Personal Health & Medical Information /Authorization
Registrars must submit these forms, along with their diocesan travel itinerary, by May 1 using this link.
Registrars must also submit the following document completed by the diocesan bishop certifying that adult chaperones meet EYE17 criteria regarding criminal background checks and completion of Safeguarding God’s Children training.
